IMPORTANT: The address needs to exactly match official documents.
Attender Details:
  • Passport copy of the attender (maximum of 2 attenders allowed per patient)
  • Attender's relationship to the patient
Nearest Indian High Commission:
  • Nearest Indian High Commission details (Location/Address details)
Treatment Information in Home Country:
  • Doctor/Hospital at hometown
  • Provisional diagnosis at hometown
  • Latest medical reports (within the last year)

IMPORTANT:
  • NO CHANGES/CORRECTIONS WILL BE POSSIBLE AFTER THE VISA INVITATION LETTER IS ISSUED BY THE HOSPITAL.
  • The hospital must upload your information to the Indian Government AYUSH website to generate the Visa Invitation Letter. Once generated, the AYUSH website does not permit the hospital to issue another letter.
  • Please ensure all provided information is accurate. Pay close attention to correctly provide details of the patient, attenders, Indian High Commission, IVAC, date extension, etc.
