I have a new born baby. Do I need to mention my baby’s name in the doctor appointment letter?

Yes. If you require your new born baby to travel with you, your baby requires a valid passport and a valid visa.

Any child (less than 16 years old) who needs to travel with you, must, be mentioned as dependent in the doctor appointment or visa invitation letter.

If I get an invitation letter under one department of Apollo Hospital, can I consult with doctors from other departments as well in the same hospital?

Yes you can. FRRO permission is required only if going to another hospital. Within the same hospital, you can visit doctors from other departments as well.

No.

Based on your current address proof, you must choose your Indian Mission address.

If your address proof is of Chittagong, you can not apply from Dhaka.

It's not mandatory for the Attender to be blood related to the Patient. However, it will be, beneficial if the attender is a relative of the patient for an Indian medical visa.

No. The date mentioned on the Visa Invitation Letter is a tentative date of appointment to facilitate your visa application.
