Pain Management

Pain Management and Treatment for Bangladeshis Worldwide

What is Pain in the Body?

Pain is a signal from your body that something is wrong. It can be sharp, dull, throbbing, or burning. You may feel it in one area, like your back, joints, or stomach, or in multiple parts at once. Pain can be short-term, called acute, or long-lasting, called chronic, and it often varies depending on where it occurs in the body. Pain can affect various body parts like

  • Head and neck: Headaches, migraines, neck stiffness.
  • Back and spine: Muscle strain, slipped disc, nerve compression.
  • Joints: Arthritis, injury-related pain in knees, shoulders, or hips.
  • Abdomen and pelvis: Cramps, digestive issues, or internal organ-related pain.
  • Chest: Can signal heart, lung, or muscle problems.
  • Limbs: Pain in arms, hands, legs, or feet due to injury, nerve issues, or circulation problems.

Pain helps protect the body by encouraging you to rest, avoid harm, or seek treatment.

Why Do People Need Pain Treatment?

People need treatment for pain to relieve discomfort, improve daily functioning, prevent the condition from worsening, and enhance overall quality of life. Left untreated, especially chronic pain, can lead to sleep problems, emotional stress, mobility issues, and reduced productivity. Early and proper pain management helps restore normal activities and supports faster recovery. Understanding the causes of pain in various body parts helps in choosing the right treatment.

Possible Causes of Pain in Neck Vein Include:

The cause of pain in the neck vein is not very common, but it can happen due to several reasons. Few common causes are:

  • Thrombophlebitis: Inflammation of a vein due to a blood clot. This may cause swelling, redness, and pain along the vein.
  • Infection: Skin infections or deeper infections like cellulitis or abscesses near the vein can cause localised pain and tenderness.
  • Jugular vein distension (JVD): Often a sign of heart problems. Though not always painful, if pressure builds, it may cause discomfort.
  • Lymph node swelling: Infections or immune reactions can cause nearby lymph nodes to press on veins, causing pain.
  • Vascular conditions: Rare conditions like venous thoracic outlet syndrome may cause vein compression and pain.

Common Causes of Pain in the Left Abdomen Include:

Why does pain occur on the left side of the abdomen? The pain can range from mild discomfort to sharp or cramping pain. Some common causes are:

  • Gas and bloating: Trapped gas in the intestines is a common, harmless cause.
  • Constipation: Hard stool can cause cramping pain in the lower left abdomen.
  • Diverticulitis: Inflammation of small pouches in the colon, often felt in the lower left side.
  • Kidney stones: Sharp, intense pain if a stone blocks the left ureter.
  • Urinary tract infection (UTI): Can cause lower abdominal pain with burning during urination.
  • Ovarian issues (in women): Conditions like ovarian cysts or ovulation pain can occur on one side.
  • Ectopic pregnancy (in women): A medical emergency if a fertilised egg grows outside the womb.
  • Hernia: A bulge or tear in the abdominal wall may cause pain, especially when lifting.

Common Causes of Arthritis Pain Include:

Arthritis pain is the discomfort, stiffness, or aching felt in joints due to inflammation. It often affects the knees, hands, hips, or spine. The pain can be mild or severe, and it may come and go or stay constant. Some common causes are:

  • Osteoarthritis: Wear and tear of the knee joint cartilage, leading to pain.
  • Rheumatoid arthritis: An autoimmune disease that causes joint inflammation and swelling.
  • Ligament injuries: Tearing of the knee ligaments, common in sports.
  • Meniscus tear: Damage to the cartilage that cushions the knee, often from twisting injuries.
  • Tendonitis: Inflammation of the tendons around the knee, often due to overuse.
  • Bursitis: Inflammation of fluid-filled sacs (bursae) near the knee joint.
  • Patellofemoral pain syndrome: Pain around the kneecap, often worsened by stairs or squatting.
  • Gout or pseudogout: Crystal deposits in the joint cause sudden, severe pain.

Causes of Knee Pain

Knee pain is a common issue that can affect people of all ages. Why does knee pain occur? It may result from injury, overuse, wear and tear, or underlying medical conditions. A few common causes are:

  • Injury: Ligament tears (like ACL), meniscus injury, fractures, or dislocation
  • Arthritis: Osteoarthritis, rheumatoid arthritis, or gout
  • Overuse: Tendonitis, bursitis, runner’s knee
  • Infection or Inflammation: Septic arthritis or joint inflammation

Common Causes of Left Breast Pain Include

Left breast pain may come and go or remain constant. While it often relates to harmless causes, it’s important to understand the possible reasons, especially when the pain is one-sided. Few common causes are:

  • Hormonal changes: Breast tenderness due to hormonal shifts is common and usually affects both breasts, but can sometimes be felt more on one side.
  • Breast cysts or fibroadenomas: Non-cancerous lumps that may cause discomfort or tenderness.
  • Mastitis: Infection of the breast tissue, often in breastfeeding women, causing pain, redness, and warmth.
  • Costochondritis: Inflammation of the cartilage where ribs attach to the breastbone, often felt near or under the breast.
  • Heart-related pain: Though not common, pain from the heart (like angina) can sometimes feel like it’s coming from the left breast. This needs immediate medical attention.

Apart from these, there are a few causes that are common to these. Muscle strain and trauma, Mechanical or physical stress, such as improper bra support, improper posture, can cause pain. Obesity is also another cause of knee pain and arthritis.

Symptoms of Arthritis Pain

Arthritis pain, especially rheumatic pain, affects the joints and surrounding tissues. Rheumatoid arthritis is a type of autoimmune disease, which means the body attacks its own joints. It often starts slowly and worsens over time. The pain may come and go or stay constant, and it can affect one or multiple joints.

Common Symptoms of Arthritis Pain (Rheumatic Pain):

  • Swelling: Joints may look puffy or feel warm to the touch due to inflammation.
  • Redness: The skin over the affected joint may become red or discoloured.
  • Reduced movement: Pain and stiffness can make it hard to bend or use the joint fully.
  • Fatigue: Feeling unusually tired even after rest is common in rheumatoid arthritis.
  • Fever or weight loss (in some cases): May occur during active inflammation, especially in autoimmune forms like rheumatoid arthritis.
  • Symmetrical pain: Rheumatic pain often affects the same joints on both sides of the body (e.g. both wrists or both knees).

Symptoms of Neck Pain

What does neck pain indicate? Neck pain is a symptom that can indicate a variety of underlying conditions, including:

  • Cervical Spondylosis: Age-related degeneration of the vertebrae and discs in the neck.
  • Herniated Disc: A disc in the neck that bulges or slips, pressing on nerves and causing pain.
  • Pinched Nerve: Compression of a nerve in the neck due to disc herniation or bone spurs.
  • Whiplash: Injury to the neck from sudden head movements, commonly from car accidents.
  • Arthritis: Osteoarthritis or rheumatoid arthritis can affect the joints in the neck, causing pain and stiffness.
  • Infections: Conditions like meningitis or throat infections can cause neck pain.
  • Heart Attack: In some cases, neck pain can be a symptom of a heart attack, especially when combined with chest pain.

Apart from these, stress can worsen arthritis and increase pain in the neck. Joint Pain and stiffness are also common symptoms of arthritis and neck pain.

Where is kidney pain felt?

Where is pain felt if there is a kidney problem? Kidney pain is typically felt in the lower back, just below the rib cage, on one or both sides of the spine. The pain may feel sharp, dull, or aching, and can radiate to the abdomen, groin, or even the sides of the lower back. It is often more noticeable on one side if only one kidney is affected, such as with a kidney stone or infection.

When Do These Symptoms Lead to Treatment?

Pain should lead to treatment as soon as it appears, especially if it is persistent, severe, or affecting daily activities. Ignoring pain can lead to worsening conditions and complications. Doctors may start treatment after:

  • A physical check-up: To assess the location, intensity, and nature of the pain.
  • Imaging tests (X-ray, MRI, or ultrasound): To detect issues like fractures, herniated discs, or internal injuries.
  • Blood tests: To identify underlying conditions like inflammation, infection, or metabolic issues that could be contributing to the pain.
  • Neurological tests: If the pain is related to nerve issues, such as sciatica or pinched nerves, additional neurological assessments may be done.
  • Joint function tests: To diagnose conditions like arthritis or gout, especially if the pain is in the joints.

What are the Basic Relief Methods for Pain?

Many effective strategies can ease both knee and back discomfort. If you’re exploring ways to reduce knee joint pain or looking for proven ways to relieve lower back pain, you’ll find that several self-care and lifestyle methods work well for both. Below are common approaches that support joint health, improve mobility, and help prevent recurring pain.

  • Rest in Moderation: Allowing time for recovery helps reduce pain, but avoid prolonged inactivity, which can cause stiffness.
  • Cold and Heat Therapy: Applying ice packs or warm compresses helps reduce inflammation and improve blood flow to the affected area.
  • Gentle Stretching: Regular stretching improves flexibility and reduces muscle tension around both joints and the spine.
  • Physical Therapy: A physiotherapist can provide exercises tailored to strengthen muscles and improve joint or spinal function.
  • Low-Impact Exercise: Activities like walking, swimming, or cycling support mobility without putting excess stress on joints or the back.
  • Weight Management: Maintaining a healthy weight reduces pressure on both the knee joints and lower spine.
  • Massage or Chiropractic Care: These therapies may help relieve pain by improving muscle relaxation and spinal or joint alignment.
  • Proper Support: Using supportive equipment like braces for knees or ergonomic chairs and mattresses for back support can help reduce strain.
  • Stress Reduction: Managing stress through relaxation techniques can reduce muscle tension and chronic pain levels.

The relief methods mentioned may not be suitable for everyone. Always consult a qualified doctor or physiotherapist before starting any new exercise, therapy, or pain management routine, especially if you have a chronic condition or severe pain.

Exercises to relieve Neck Pain

Here are some simple and effective exercises to relieve neck pain, which can help reduce tension, improve mobility, and strengthen the neck muscles:

1. Neck Tilts (Forward and Backward): Sit or stand straight. Slowly tilt your head forward, bringing your chin to your chest. Hold for 5 seconds, then return to the starting position. Tilt your head backward, looking up toward the ceiling. Hold for 5 seconds. Repeat: 5-10 times.

2. Side-to-Side Neck Stretch: Slowly tilt your head toward your right shoulder (without raising the shoulder). Hold for 10-15 seconds. Repeat on the left side. Repeat: 3 times on each side.

3. Neck Rotation: Turn your head slowly to the right until you feel a gentle stretch. Hold for 5 to 10 seconds, then return to the centre. Repeat on the left side. Repeat: 5-10 times per side.

4. Shoulder Roll: Roll your shoulders in a circular motion forward 5 times, then backward 5 times. It relieves tension in the neck and upper back.

5. Chin Tucks: Sit or stand with your back straight. Gently tuck your chin in, as if making a double chin. Hold for 5 seconds, then release. Repeat: 10 times.

6. Upper Trapezius Stretch: Sit on your right hand. With your left hand, gently pull your head toward your left shoulder. Hold for 20 seconds. Switch sides. Repeat: 2-3 times on each side.

Always consult a doctor or physiotherapist before starting any exercise if you have a serious neck condition.

Cost of Pain Treatment

On average, pain treatment in India ranges from $1100 to $3300, while in Thailand it is from $4000 to $6000. The final cost may vary based on multiple factors such as the treatment method, hospital location, and severity of pain. Before viewing the detailed table of treatment-wise costs, it's helpful to understand what influences these expenses most.

Factors Affecting Pain Treatment Cost

Several factors can influence how much pain management will cost. These include:

  • Type of pain: Simple muscle pain or back pain is cheaper to treat than chronic nerve pain, arthritis, or spinal conditions.
  • Treatment method: Medication and physiotherapy are low-cost, while injections, nerve blocks, or minimally invasive procedures cost more.
  • Severity and duration: Chronic or long-term pain usually requires multiple sessions and ongoing care, increasing the total expense.
  • Diagnostic tests: X-rays, MRIs, CT scans, or nerve studies may add to the cost depending on the pain’s cause.

Success & Outcomes for Pain Treatment

Success in pain management means reducing or removing pain, improving daily activity, and helping the patient return to a better quality of life without constant discomfort.

Understanding Success Rates

The long-term survival of rheumatoid arthritis (RA) patients is influenced by early diagnosis, timely intervention, and the choice of treatment strategy. Initiating appropriate therapy early in the disease course is crucial for improving outcomes and reducing mortality. When RA is managed promptly with biologic or targeted synthetic DMARDs (b/tsDMARDs), patients experience significantly better long-term survival.

  • In this study, patients receiving b/tsDMARD therapy for at least six months had an overall survival rate of 87.3% over a mean follow-up of 9.4 years.
  • The mortality rate observed was 13.5 deaths per 1,000 treatment-years, reflecting favourable outcomes in a real-world cohort.

What Does Success Mean?

  • Significant relief from chronic or severe pain
  • Improved movement and ability to do daily activities
  • Reduced need for long-term medication
  • Fewer flare-ups or pain attacks over time
  • Better mental and emotional health due to pain control

If the pain is diagnosed early and treated with the right approach, most patients feel much better and can manage their lives with comfort. Regular therapy, proper posture, and timely medical support help maintain long-term results.

Approach of Leading Hospitals

Top hospitals focus on accurate diagnosis, advanced therapies, and holistic care for patients with pain and related symptoms. Their approach includes:

  • Comprehensive diagnostics such as physical assessments, imaging tests (X-ray, MRI, CT), nerve studies, and blood tests to identify the underlying causes and severity of pain.
  • Modern treatments including targeted pain medications, nerve blocks, physiotherapy, integrative therapies, palliative interventions, and minimally invasive procedures for chronic pain relief.
  • Supportive care with counselling, lifestyle guidance, rehabilitation, and long-term monitoring to improve mobility, comfort, and quality of life.
  • Multidisciplinary reviews, where pain specialists, palliative care doctors, physiotherapists, psychologists, and nutritionists work together to design personalised care plans.

This combination of advanced testing, expert management, and patient-centred care helps reduce pain, ease symptoms, and restore daily functioning for patients with acute or chronic conditions.

Dr Vijayan explains pain types and holistic pain management at Manipal Hospital, Bangalore

According to Dr Vijayan, Manipal Hospital, Bangalore, pain is more than just a physical sensation; it’s both a sensory and emotional experience tied to actual or potential tissue damage. This means pain affects both the mind and body, varying greatly from person to person.

Pain is broadly classified based on duration and cause. Acute pain lasts less than three months and typically follows surgery, injuries, or trauma, subsiding as healing occurs. In contrast, chronic pain persists beyond three months and often stems from long-term changes in the spinal cord or brain. From a pathological perspective, nociceptive pain results from tissue injury, while neuropathic pain arises from damage or malfunction within the nervous system.

Dr Vijayan highlights that treatment involves a combination of medications, interventional procedures, physiotherapy, and counselling, all aimed at restoring comfort, function, and quality of life.

Dr. Anand Murugesan discusses chronic pain causes, early diagnosis, and advanced pain management strategies

Dr. Anand Murugesan, Pain Management Specialist at Apollo Hospitals, Chennai, highlights that chronic pain, especially back pain, neck pain, shoulder pain, and headaches, are increasingly common due to modern lifestyle habits like excessive smartphone and laptop use, poor posture, and sedentary living. He explains that when pain lasts more than three months, it becomes chronic and far more complex to treat because it affects not just the body but also a person’s psychological, emotional, and occupational health. Chronic pain management, therefore, is highly challenging and requires a multi-dimensional approach.

Dr. Anand emphasises the importance of early diagnosis. His core message is clear: chronic pain should not be ignored, and early treatment can greatly improve quality of life.
